. Supplementary Information Report No.: 50-302/al-080/03L-0 Facility: Crystal River Unit 3 Report Date: January 15, 1982 Occurrence Date: December 21, 1981 Identification of Occurrence:

Main Steam Valve 130 was inoperable contrary to Technical Specification 3.6.3.1.

Conditions Prior to Occurrence:

Mode 1 (100%),

Description of Occurrence:

At 0440 during normal operation, Main Steam Valve 130 would not operate after packing adjustment was made to stop excessive leakage,under Work Request #29981. At 0615 Main Steam Valve 176 was closed to isolate Main Steam Valve 130 and maintenance was initiated. Operability was restored at 1748 on December 22, 1981 Designation of Apparent Cause:

The cruse of this event is at.tributed to excessive packing adjustment which caused the valve to become inoperable.

Analysis of Occurrence:

There was no effect upon the health or safety of the general public.

Corrective Action:

The valve was repaired under Work Request #29981, m.d operators will be instructed in valve packing adjustment. No further corrective action is deemed necessary.

Failure Data:

This was the second occurrence of this type, and this is the thirty-seventh event reported under this Specification.
