SUPPLEMENTARY INFORMATION

{ Report No.: 50-302/81-076/03L-0 Facility: Crystal River Unit 3 Report Date: December 23, 1 981 Occurrence Date: December 5, 1981 Identification of Occurrence:

Reactor Protection Instrument NI-6 would not properly function during shutdown surveillance activities. This event was not attributed to a specific shutdown activity as required by Regulatory Guide 1.16, and, consequently is contrary to Technical Specification 3.3.1.1.

Conditions Prior to Occurrence:

During the performance of SP-113, Power Range Nuclear Instrumentation Calibration, the bistable for NI-6 in Reactor Protection System Channel "B" would not trip.

Maintenance was initiated and operability was restored.

Designation of Apparent Cause:

The cause of this event is attributed to a relay failure.

(The bistable was Bailey Meter Co. part #6621500-DIY-1A, and the relays were #HG-52MT-5105).

Analysis of Occurrence:

There was no effect upon the health or safety of the general public.

Corrective Action:

The relay was replaced. No further corrective action is deemed necessary.

Failure Data:

This was the first occurrence of this type, and this is the tenth event reported under this Specification.
