, Occurrence Date: November 27, 1981 Identification of Occurrence:

Failure to meet Technical Specification 3.3.3.3, that all-seismic monitoring instrumentation is operable.

Conditions Prior to Occurrence:

Mode 5 (0%).

Description of Occurrence:

At 0322, during performance of SP-155, Channel Check of the Triaxial Peak Accelographs, it was discovered that accelo-graph SI-5-MEI, did not meet the requirement that all seismic monitoring instruments are to be operable as per Technical Specification 3.3.3.3. The unit was replaced and operability was restored on December 3, 1981.

Designation of Apparent Cause:

The cause of this event is attributed to a rigorous environ-ment and age.

Analysis of Occurrence:

There was no effect upon the health or safety of the general public.

Corrective Action:

Accelograph SI-5-MEI was replaced and SP-155 was performed satisfactorily. Further corrective action is deemed unnecessary.

Failure Data

This is the first occurrence for accelograph SI-5-MEI, and this is the second report under this Technical Specification.
