2A Safeguards Equipment Cabinet - Inoperable.

This response was initiated by Incident Report 81-471.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - dx Power 94% - Unit Load 1071 MWe f

DESCRIPTION OF OCCURRENCE:

On November 19, 1981, after the satisfactory performance of the M3B sur'veillance test, the 2A Safeguards Equipment Cabinet (SEC)

'would not acknowledge being reset. The M3B surveillance was performed a second time satisfactorily. Upon the attempt to reset,the 2A SEC, 2A vitel' bus tripped, stripping the bus of i ts loads. At 1154 hours0.0134 days <br />0.321 hours <br />0.00191 weeks <br />4.39097e-4 months <br /> 2A'SEC was declared operable and Action Statement 3. 3. 2 Action 13 was entered.

Th2s occurrence constituted operation in a degraded mode in a,ccordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

2A SEC did not reset due to the failure of the SK6 input reset relay. <

ANALYSIS OF OCCURRENCE:

Technical Specification 3.3.2 Action 13 requires:

LER 81-123/03L CORRECTIVE ACTION:

The SK6 input reset relay was replaced. 2A SEC reset and tested satisfactorily. At 1456 hours0.0169 days <br />0.404 hours <br />0.00241 weeks <br />5.54008e-4 months <br />, November 19, 1981 2A SEC was de-clared operable and Action Statement 3.3.2 Action 13 was terminated.

During the time period 2A SEC was declared inoperable, the plant was adequately protected by 2B and 2C SEC.
