Report Number: 81-ll2/03L Report Date: 11-25-81 Occurrence Date: 10-28-81 Facility: Salem Generating Station, Unit 2 Public Service Electric & Gas Company Hancocks Bridge, New Jersey 08038 IDENTIFICATION OF OCCURRENCE:

100' Elevation Containment Air Lock - Inoperable.

This report was initiated by Incident Report 81-430.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 99% - Unit Load 1130 MWe DESCRIPTION OF OCCURRENCE:

On October 28, 1981, the 100' Elevation Containment Air Lock was found to be inoperable. Action Statement 3.6.1.3 was entered at 1500 hours0.0174 days <br />0.417 hours <br />0.00248 weeks <br />5.7075e-4 months <br />.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of the air lock being inoperable was the inside door seal had fallen out of the groove.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.6.1.3 requires:

With an air lock inoperable, restore the air lock to operable status within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> or be in at least hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in cold shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

CORRECTIVE ACTION:

The inside door seal was replaced, and the air lock leak rate test was performed satisfactorily. At 1658 hours0.0192 days <br />0.461 hours <br />0.00274 weeks <br />6.30869e-4 months <br />, October 28, 1981 the air lock was declared operable and Action Statement 3.6.1.3 was terminated.
