SUPPLEMENTARY INFORMATION

, Report No.: 50-302/80-055/03L-1 F:cility: Crystal River Unit 3 Report Date: June 11,1981 Occurrence Date: December 18, 1980.

Identification of Occurrence:

Failure of the Engineered Safety Feature Actuation System Instrumentation Channels contrary to Technical Specifica-tion 3.3.2.1.

Conditions Prior to Occurrence:

Mode 1 power operation (99%).

Description of Occurrence:

At 0846 during performance of SP-130. Engineered Safeguards Monthly Functional Test, RC-3 pressure high channel load sequence block 3 trip circuit would not reset following actuation test. This event was evaluated to require compli-ance with Action Stctement #10. At 1042 it was determined, after teevaluation,that Action Statement #9 also applied; the inoperable channel was tripped at 1059. Maintenance was initiated and operability was restored at 1310.

Designation of Apparent Cause:

The cause of this event was isolated to relay 62-2/RC3.

It was an Agastat type 3722.

Analysis of Occurrence:

There was no effect upon the health or safety of the general public.

Corrective Action:

The relay was removed,and a bench check was satisfactory.

The relay was reinstalled,and the functional test was satisfactory. Surveillance frequency was doubled,and the problem was determined to be a loose connection in the relay mounting block. This revision is to provide relay model number and to update previously committed corrective action.

Failure Data:

This is the first occurrence for load sequence block 3 trip circuit, and the fifteenth event reported under this Specification.
