~0ccurrence Date: February 19, 1981 Identification 'of Occurrence:

Failure of Containment Isolation Valves MUV-260 and MUV-261 to close during normal plant cooldown. This created an event not attributed to a specific shutdown activity as required by Regu-latory Guide 1.16 and therefore, contrary to Technical Specifica-tion 3.6.3.1.

Conditions Prior to Occurrence:

Mode 5 cold shutdown. (0%)

Description of Occurrence:

At'1500 during normal plant cooldown Containment Isolation Valves MUV-260 and MUV-261 failed to close. Maintenance was initiated and operability was restored on February 24, 1981.

. Designation of Apparent Cause:

P The cause of MUV-260 failing.to close is unknown. Investiga-tion of the event did not reveal any apparent reason for failure; the event could not be duplicated. MUV-261 torque switch failed.

Analysis cf Occurrence:-

There was no effect upon the health or safety of the general public.

Corrective Action:

MUV-261 torque switch was replaced. An engineering evaluation of Limitorque Operated Containment Isolation Valves is in progress.

Failure Data:

This was the first occurrence for MUV-260 and for MUV-261 and this is the thirty-fourth event reported 'under this Specifica-

tion.
