, Report Date: 5 November 1980 Occurrence Date: 5 September 1980 (discovered at plant 10 October 1980) r

, Identification of Occurrence:

Contrary to Environmental Technical Specification 3.1.5 pH results were not reported for the waste water ponds, test wells and canals.

i Conditions Prior to Occurrence: N/A l Description of Occurrence:

The pH samples from the ponds, wells and caaals were not

reported, even though they were analyzed, for September 1980 l

because the results were misplaced.

Designation of Apparent Cause:

4 The non-availability of pH results was due to personnel wha performed the analysis losing the data.

Analysis of Occurrence:

The noncompliance did not result from the operation of Crystal River Unit #3, nor is operation of the plant affected by it. There was no effect upon the general public health or safety.

I Corrective Action:

Personnel performing analyses were admonished to follow procedures precisely. Additionally, samples will be retained for an adequate time to allow reanalysis if data'is lost.
