Catawba Nuclear Station Units 1 and 2 IDENTIFICATION OF DEFICIENCY:
Improper operation of optical isolator model 175Cl41.
INITIAL REPORT: On August 28, 1980, Mr. John Rausche of.9egion II was notified of this deficiency by Mr. J. R. Wells and'Mr. J. E. Thomas of Duke Power Company, Charlotte, NC 28242.
SUPPLIER / COMPONENT: The malfunctioning optical isolators were part number 175C141, supplied by E-max Instruments, Inc., Englewood, Colorado, in accordance with Duke Power Specification: CNS-1338.00-00-0001.
l DESCRIPTION OF DEFICIENCY: During operation and testing a number of the subject

isolators _ failed to operate properly when driving loads with greater than j
one amp inrush current.
It has been determined that the one amp inruth t
current exceeds the capacity of this model isolator. This deficiency potentially affects approximately 57. of the isolators installed at the l
Catawba Nuclear Staion.
SAFETY IMPLICATIONS: Failure of the isolator output to correctly respond to the input could prevent proper operation of a safety function of that circuit. However, the isolation capability of these devices is not affected by the described deficiency.
CORRECTIVE ACTION: All model 175C141 isolators driving loads with excessive inrush current will be changed out with isolator part number 175Cl46, of adequate current capacity. This change is scheduled to be completed by 1/1/82.
