DISCOVERED July 7, 1980 DESCRIPTION OF NONCONFORMANCE, ANALYSIS OF CONDITION, AND CORRECTIVE ACTION (ATTACHSUPPORTINGDATA).
A~ possibility exists that in a " worst case" tolerance buildup condition, the gate. could disengage. from the guide when valve is in the backseat position. -
A stem extension bushing has been designed and machined that !
will interface between the stem and the backseat. When installed, it will prevent any possiblity of disengagement of the gate from the guide.
A modification procedure has been written. Valves will be modified in the field. All new valves will incorporate a changed gate guide location to prevent recurrence.
