4. Occurrence Date: 23 July 1978

5. Identification of Occurrence:

Receipt of contaminated spent fuel cask and reported in accordance with the provisions of 10 CFR 20 Patre 205(b)(2).

6. Conditions Prior to Occurrence:

Mode 6 refueling.

7. Dascription of Occurrence:

At 1205, NAC spent fuel cask # USA-66981/3F was received on site with a removable radioactive contamination in excess of 22,000 DPM. I:nmediate isolation within a radiation controlled area and decontamination of the- cask was initiated.

The decontamination of the cask was completed on 23 July 1978. There was no smearable contamination on the transport vehicle.

.8 b ,signation of Apparent Cause-The cause of this event is undetermined as the cask and vehicle had been intransit from several locations during the preceding week.

9. Analysis of occurrence:

There was no known safety hazards to the general public and i:nmediate isolation and decontamination was initiated upon discovery at this location.

10. Correceive Action:

Corrective action to preclude recurrence cannot be initiated at this site.

11. Failure Data:

This is the first occurrence of an event of this type.
