5. Identification of occurrence:

Automatic actuation of the "B" steam line rupture natrix contrary to Table 3.3-3 of Technical Specification 3.3.2.1.

6. Conditions Prior to Occurrence:

Mode 3 operation.

7. Description of Occurrence: l At 1930, during plant heatup and prior to entry into operational Mode 2, the "B" 5 steam line rupture matrix actuated at approxima':aly 760 psig. The 36 hour4.166667e-4 days <br />0.01 hours <br />5.952381e-5 weeks <br />1.3698e-5 months <br /> Action Statement was entered in accordance with action #10 of the Tech Spec. Feedwater valves WV-28 and 39 were manually opened to re-establish flow to the "3B" Once Through Steam Generator. Maintenance personnel were notified and Work Raquest b4 #T-1794 was initiated. Maintenanca restored operability of the "B" steam line V rupture matrix and the Action Statement was satisfied at 2355. 1
8. Designation of Apparent Cause:

This event was due to an upward drift of the " Dead Band" of the 600 psig bypass pressure switches in that the switches did not reset from previous cooldown.

9. Analysis of Occurrence:

There was no safety hazard to the plant or general public as a result of this occurrence. l l

10. <rrective Action:

Pressure switches MS-94, 95-PS2 were re-calibrated and a Request for Engineering Informa-tion was submitted to install manual bypasses so that each of the matricies can be by-passed, but interlocked so only one matriz can be bypassed at any one time.

11. Failure Data:

This is the first occurrence of this event.
