5. Identification of Occurrence:

Auxiliary Building exhaust fans 14 B & D (AHF-14 B&D) inoperable contrary to Technical Specification 3.9.12.

6. Conditions Prior to Occurrence:

Mode 6 refueling.

7. Descriptien of occurrence:

At 1900 hours0.022 days <br />0.528 hours <br />0.00314 weeks <br />7.2295e-4 months <br /> it was discovered that the Auxiliary Building exhaust ventilation was reversed creating a positive pressure in the Auxiliary Building for I hour and 10 minutes. Investigation by maintenance personnel revealed that the dampers to the Auxiliary Building fans 14 B&D (AHF B&D) failed to open because of a burned-out control solenoid. The solenoid was renewed and AHF-14 B&D was returned to service.

8. Designation of Apparent Cause:

The cause of this event was due to a burned-out damper control solenoid.

9. ' Analysis of Occurrence:

The health and safety of the plant and public was not endangered, as no detectable particulate or noble gas was discharged.

10. Corrective Action:

No corrective action necessary as the control solenoid was renewed.

11. Failure Data:

First occurrence of an event of this type.
