Dear Mr. Grier:

In accordance with Technical Specification 6.9.1.8.1, the following occurrence is reported. The report was discussed with Mr. D. R. Haverkamp at 1615 hours0.0187 days <br />0.449 hours <br />0.00267 weeks <br />6.145075e-4 months <br /> on March 5, 1979.

On March 3, 1979 during maintenance activities on the 1B trip valve to the turbine driven steam generator auxiliary feedwater pump, a nut and washer were discovered in the line. An investigation was commenced to determine the origin of the components. On March 5, 1979, it was determined the nut and washer may have come from one of the three check valves upstream of the trip valve. At 1828 hours0.0212 days <br />0.508 hours <br />0.00302 weeks <br />6.95554e-4 months <br />, the turbine driven steam generator auxiliary feedwater pump was isolated and declared inoperable.

The three upstream check valves were disassembled and inspected. The nut and washer on the 1C steam generator steam supply to the aux 111'ary feedwater pump check valve were missing. The uuts and washers on the 1A and 1B check valves were intact. ,

Any additional information and corrective action will be included in the followup report.
