SUPPLEMENTARY INFORMATION Report No.: 50-302/79-015/03L-0 Facility: Crystal River Unit #3 Report Date: 9 March 1979 Occurrence Dates: 21 February 1979 and 8 March 1979 Identification of Occurrence:

The fire suppression water system inoperable contrary to Technical Specification 3.7.11.1.b.

Conditions Prior to Occurrence:

Mode 1 power operation (98%),and Mode 5 cold shutdown (21 February 1979) (8 March 1979)

Description of Occurrence:

At 0300 on 21 February 1979, while performing daily Operating Surveillance Procedure, SP-300, it was reported that the fire service tank FST-1B volume was 340,000 gallons. The seven day action statement entered. The fire service tank FST-1B was filled to within the Technical Specification limit at 0700 21 February 1979.

At 0900 8 March 1979, fire service tank FST-1A was reported to be less than the minimum volume required by Technical Speci-fications. The fire service tank FST-1A was filled to within the Technical Specification limit at 1515 on 8 March 1979.

Designation of Apparent Cause:

The cause of these events is attributed to improper design.

The low level alarm was found installed below Technical Speci-fication level requirements.

Analysis of Occurrence:

No hazard to public health and safety as the alternate fire service tank in each event was available as well as the makeup from the adjacent Units 1&2.

Corrective Action:

Modification Approval Record 79-3-63 has been initiated to change the fire service tank low level alarms to approximately twelve inches above the Technical Specification limit. An additional engineering evaluation has been initiated to determine accurate tank volume that will provide accurate tank level indication.

First and second occurrences reported under this Technical Spect-fication.
