SUPPLEMENTARY INFORMATION Report No.: 50-302/79-068/03L-0 Focility: Crystal River Unit #3 Report Date: 15 August 1979 Occurrence Date: 23 July 1979 Identification of Occurrence:

Failure to perform post maintenance testing of core flood valves CFV-15 and CFV-16 contrary to Technical Specification 4.6.3.1.1.

Conditions Prior to Occurrence:

Mode 4 hot shutdown.

Description of Occurrence:

At 1630 it was reported that post maintenance testing had not been completed on CFV-15 and CFV-16 prior to returning the valves to service. Further investigation revealed that CFV-15 and 16 were tested satisfactorily following main-tenance on 21 July 1979, but procedural controls were not available to document the testing results. CFV-15 and 16 were successfully retested per an amended Quarterly Cycling of Valves Procedure SP-370, and returned to service at 1520 on 23 July 1979.

Designation of Apparent Cause:

The cause of this event is attributed to defective procedures.

SP-370 did not contain provisions for testing CFV-15 and 16.

Analysis of Occurrence:

No effect upon the plant or general public as the plant remained in a safe shutdown condition.

Corrective Action:

A revision has been implemented to include CFV-15 and CFV-16 in Quarterly Cycling of Valves Procedure SP-370.

Failure Data:

This is the first occurrence of this type reported.
