SUPPLDfENTARY INFORMATION Report No.: 50-302/79-059/03L-0 Facility: Crystal River Unit 3 Report Date: 03 July 1979 Occurrence Date: 13 June 1979 Identification of Occurrence: Failure to perform surveillance procedure SP-179, Containment Leakage Tests B & C, within the required time interval of Technical Specification 4.6.1.2.d.

Conditions Prior to Occurrence: Mode 5 cold shutdown.

Description of Occurrence: At 1530 it was determined that surveillance procedure SP-179, Containment Leakage Tests B & C, had exceeded the due date of 18 November 1978. SP-179 was scheduled and completed with satisfactory results 19 June 1979.

Designation of Apparent Cause: The ca 3e of this event is attributed to personnel error. A 25% extension for completion of the surveillance procedure was inadvertently applied to the 24 month time interval of SP-179.

Analysis of Occurrence: No effect upon public health and safety as the plant remained in a safe shutdown condition.

Corrective Action: The limitations of surveillance procedures with definite time intervals were discussed with the responsible individual.

Failure Data: This is the first occurrence of this type reported.
