. 02/21/78 A. GENERAL

1. Purpose.

The purpose of this procedure is to establish methods to insure that bypassing of reactor safety functions, installation of electrical jumpers and lifting of electrical and temporary mechanical modifications will be properly controlled, identified, and recorded in the appropriate control room logs.

2. Scope.

This procedure provides the requirements for control and documenting the following functions:

a. Use of the bypass features listed in Enclosure I and V.
b. Lifting of electrical leads or opening links in an electrical circuit.
c. Installation of electrical jumpers.
d. Installation or removal of blank flanges that are not a design part of the system.
e. Use of spool pieces or temporary lines that are not a design part of the system.
f. Evaluating lifted leads, jumpers and temporary mechanical modifications left longer than 12 months.

This procedure does not include spool pieces or blank flanges that are a design part of the system that are installed and removed by operating, maintenance, or surveillance procedures.

Pipe caps or blank flanges provided to stop vent or drain valve leakage where the cap (flange) is visable from the vent (drain) valve is likewise not included.

B. RESPONSIBILITIES

1. Maintenance Department.
a. Maintenance personnel shall be thoroughly familiar with this procedure in order to insure that operations personnel are proplerly notified prior to placement of any electrical jumper, lifting of electrical leads, placement of temporary spool pieces, blank flanges or other temporary mechanical modification.

, b. Maintenance personnel shall be responsible for the installation of electrical jumpers, lifting of leads, installation or removal of flanges, spool pieces oc other temporary mechanical modifications.

c. Maintenance personnel shall be responsible to insure that the jumper used is compatible with its intended function insofar as wire size, termination, insulation, etc. are concerned.
d. Maintenance supervision shall be responsible for instructing their main-tenance personnel .in the conduct of this procedure.
e. Maintenance personnel shall use a jumper of appropriate size & length for the job. However -a jumper shall be installed until it has been identified by tags,obtained in the control room, placed upon the jumper at each end, and each tag bearing the same No. All jumper tags shall be RED in color.

Use of these tags shall be recorded in the jumper log by number.

f. Maintenance personnel shall place a tag on every lifted land or sliding link. Each tag shall bear a number and shall be BLUE in color. When ob-taining the tag in the control room a record of its use shall be made in the lifted lead log by numbers.
g. Maintenance personnel shall place a metal or bakelite numbered tag on all spool pieces or blank flanges that are not in their normal operating con-dition or position, and "on" all mechanical jumpers or modifications. The reasoa for this shall be logged in the Mechanical Modification Log in the <

control room when picking up the numbered tags.

2. Operations Department
a. Operating personnel will be responsible for performing the bypassing of safety functions and the proper recording thereof.
b. Operating personnel will be responsible for overall control of electrical jum,.er installations, lifted lead conditions, and temporary mechanical modifications.
c. Operations supervision will be responsible for training operations personnel in the coiduct of this procedure.
d. Key control for the control of key switches associated with bypassing safety function: shall be the responsibility of the operations uepartment and 1122 246

, .02/2T/78 shall be performed in accordance with Administrative Procedure 1011 (Controlled Key Locker Control).

3. Engineering Department
a. Engineering personnel having cognizance over the jumper or lead shall perform an evaluation on those in effect longer than 12 months. The evaluation should identify the reasons l

why the wiring change has not been made permanent and what final disposition of the situation is intended.

C. LIMITS AND PRECUATIONS

1. Bypassing of Safety Functions.
a. The bypassing of a safety function shall in no way jeopardize Unit safety or violate TMI's Unit 1 or Unit 2 Technical Specifications or operating documents.
b. Bypassing of safety functions will normally be performed during routine Unit start-up and shutdown operations, periodic surveillance testing or other specific modes of operations or maintenance as dictated by the applicable Metropolitan Edison documents (E.G. operating procedures, maintenance procedures, surveillance procedures, etc.).
2. Jumper and Lif ted Leads Control.
a. In no case will temporary electrical jumpers be installed or leads liftec without the consent of the Shift Foreman.

Permanent wiring changes do not fall within the scope of this procedure.

3. In no case will temporary mechanical modifications be made without the consent of the Shift Foremen. Permanent piping or equipment changes do not fall within the scope of this procedure.

1. Bypass of Safety Function.
a. This procedure addresses itself to bypassing of safety functions in the following systems:

(2) Engineered Safeguards Actuation System (Unit 1).

(3) Safety Features Actuation System (Unit 2).

(4) Control Rod Drive System.

(5) Radiation Monitoring System.

(6) Emergency Feed Water System.

(7) Main Feed Water System.

(8) Turbine lift pumps pressure switch bypass.

(9) Unit 1 E.S. Breaker control key-operated bypass switches (69).

(10) Unit 2 E.S.F. Breaker control permissive switches (69),

b. The bypassing shall normally be accomplished by the control room operator by means of the bypass switches listed on Enclosure 1 (Unit I), or Enclosure V (Unit 2).

The safety bypass switch enclosures identify the switch bypass functions and switch locations under the scope of this procedure for the applicable unit.

c. The Unit 2 permissive switches (69) operating handles shall be removed from the switchgear and controlled by the Shift Foreman.
d. When it is determined that the bypass should be in effect and all system prerequisites have been met per the applicable operating, surveillance, and/or maintenance procedures, the control room operator will initiate the bypass and enter the following data in the _ control room operator's log. The Shift Foreman will also enter this information in the shift foreman's log.

e. If the bypass is other than a normal . plant bypass function and requires the installation of an electrical jumper, an appropriate entry should also be made in the jumper and lifted leads control log (refer to paragraph D.2.c).
f. Whenever a bypass of a safety function is removed, the control room operator and the Shift Foreman will make the following entry in their respective logs:

(1) Bypass function title.

(2) Date and time bypass is removed.

(3) Reason for removing the bypass.

2. Electrical jumper, Lifted leads and Temporary Mechanical Modifications Control
a. This procedure addresses itself to the control and recording of installation of electrical jumpers, lifted leads, and temporary mechanical modifications.

b. A log bearing the title " JUMPER, LIFTED LEADS AND TEMPORARY MECHANI-CAL MODIFICATIONS CONTROL LOG" shall 'e retained in the control room by the control room operator. Entries in the log will be made by the person performing the operation or his designee and initialed by the Shift Foreman prior to placing the jumper, lift-ing a lead or making a temporary mechanical modification.
c. Three types of log sheets will be used in the log book depending upon the nature of the work to be done. The Jumper Log Sheet (ENCLOSURE II) will be used when placing an electrical jumper, the Lifted Leads Log Sheet (ENCLOSURE III) will be used when lifting electrical leads, and the temporary Mechanical Modifica-tion Log Sheet (ENCLOSURE IV) will be used when making a temporary mechanical modification.
d. Whenever jumpers are placed the following information shall be filed in the log:

3. Evaluation of Lifted Leads, Jumpers or Temporary Mechanical Modifications in Effect Greater than 12 Months.
a. When a lifted lead, jumper, or temporary mechanical modification is in effect greater than 12 months it snould be evaluated by the cognizant engineer for the affected system. tiormally, the audit of the log book will identify this and a cognizant engineer should be notified.
b. The evaluation should document the reasons why the wiring change has not been mada pe:manent (or removed) and what final disposition of the situation is intended.
c. A copy of this written evaluation should be attached to the log sheet for the applicable lifted lead, jumper, or temporary mechanical modification until tre item is closed out or a permanent change is implemented.
