Inoperable Reactor Trip Instrumentation, Reactor Coolant Loop 13 OTt:.T CONDITIONS PRIOR TO OCCURRENCE:

Operational Mode 1

'Reactor Power: 88% increasing to 100%

DESCRIPTION OF OCCURRENCE:

At 0650 hours0.00752 days <br />0.181 hours <br />0.00107 weeks <br />2.47325e-4 months <br />, while increasing reactor power to 100%, the "Over-temperature t:.T. Block Rod Withdrawal and Start Turbine Runback" alarm was received in the control.room. Loop 13 OTt:.T.channel had failed. The control operator blocked loop 13 t:.T and Tavg Deviation and declared loop 13 OTt:.T channel inoperable. Technical Specifica-tion 3.3.1.1 Action Statement No. 2 was implemented and .the OTt:.T channel was placed in the tripped condition.. Instrumentation per-sonnel investigated and reworked loop 13 OTt:.T channel such that the channel was returned to service at 1530 hours0.0177 days <br />0.425 hours <br />0.00253 weeks <br />5.82165e-4 months <br />. Action State-ment No. 2 for LCO 3.3.1.1 was then terminated.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The.cause* of this occurrence was the failure of a Hagan Function Generator. The function generator failure resulted from a failed capacitor. The failure of this capacitor reduced the OTt:.T set-point to the actual t:.T value which is in the conservative direction.

ANALYSIS OF OCCURRENCE:

Technical Specification Action Statement No. 2 for LCO 3.3.1.l states that with the number of operable channels one less than the total number of channels, startup and/or power operation may proceed provided the following conditions are satisfied:

a) The inoperable channel is placed in the tripped condition within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />.

b) The minimum channels operable requirement is met.

c) Either, Thermal Power is restricted to < 75% of Rated Thermal Power and the Power Range, Neutron Flux-Trip Setpoint is reduced to < 85% of Rated Thermal Power within 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br />; or, Quadrant Power Tilt Ratio is monitored at least once per 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

CORRECTIVE ACTION:

The failed function generator was taken to the instrumentation repair shop where investigation revealed the failed capacitor. The capacitor was replaced and the unit was bench tested with satisfactory results. The function generator was returned to service in the loop 13 OT~T channel. Functional testing confirmed operability.
