I I I On November 12, 1986,. in response to NRC IEIN 86:...53 concerning the improper installat:i,.on of Raychem splice material, an inspection of the Environmentally Qualified (EQ) equipment was performed~ At the time of the inspection, Unit 2 was in cold shutdown. The inspection discovered a number of EQ components that were not installed per approved Raychem guidelineso Procedures used during the initial installation of the Raychem heat shrink tubing did not conform to the Raychem Application Guide which resulted in improper installation. A contributing factor was inadequate training of personnel installing the heat shrink material.

The discrepancies identified were corrected. The procedures have been revised to.reflect the current application guide and additional training has been conducted.

At the time of the inspection Unit 2 was in the cold shutdown condition.

The inspection discovered a number of Raychem splices on. EQ equipment that existed in the following general unqualified configurations.

1) *Raychem not installed per the manufacturers instructions.
2) Raychem installed over non impregnated braided cable or was heated such that blistering occurred.
3) The termination was required to be reheated to firmly seal the Raychem.

2.0 Safety Consequences and Implications Equipment associated- with the subject Raychem splices included direct acting solenoid trip valves, solenoid operated trip valves (SOV's), and instrumen-tation such as reactor coolant system temperature, pressure and level transmitters and containment sump level transmitters.

The trip valves are designed to ensure containment isolation during an event. Since these valves are designed to fail closed, an electrical failure would result in the required isolation. Also, redundant instrumentation for the parameters noted above, remained unaffected arid would have been used in lieu of the affected instrumentation during an event.

. . . I Other items identified, associated with the Raychem splices, were the 'A' & 'B' inside recirculation spray pumps (IRS) {EIIS-BE} and the*, 'A' & 'B' charging pump {EIIS-BQ}. The. IRS pump motor lead splices were found to exist in configuration (2) with Raychem over non impregnated braided cable, and the charging NRC FOl'IM 388.A

( 1) . Specifically, 'A-' charging pump splice had been constructed without an inner sleeve and 'B' charging pump splice was found with tape underneath the Raychem

3.0 Cause Procedures used during the initial installation of the Raychem-heat shrink tubing.did not conform to the Raychem Application Guide, resulting* in improper instalia-tion. A contributing factor was inadequate training of personnel installing the heat shrink material.

4.0 Immediate Corrective Actions None required.

5.0 Additional Corrective Actions The discrepancies identified were corrected.

6.0 Actions Taken*to Prevent Recurrence The procedures have been updated to reflect the current Raychem Application Guide *. Additional training to personnel installing the Raychem material has been conducted. *
