  • SURRY POWER STATION, UN 2 DOCKET NO: 50-281 REPORT NO: 80-017/03L-0 EVENT DATE: 08-20-80 TITLE OF REPORT FLOW TRANSMITTER (FT-2495) FAILURE
1. Description of Event:

During startup operation with Unit #2 at 18% power, control room surveillance revealed a zero reading on main steam flow indicator (FI-2495) powered from main steam flow transmitter (FT-2495). This is a degraded mode of operation as permitted by T.S.3.7.A and is reportable in accordance with T.S.6.6.2.b(l).

2. Probable Consequences and Status of Redundant System:

With this transmitter not fully functional, there was no steam line flow in-dication for one of the two channels that monitor and provide steam line break protection for "C" Steam Generator. The redundant steam line flow transmitter for this Steam Generator was still available to provide the required control and protection signals. In addition other diverse protection signals were available to mitigate the consequences of the main steam pipe rupture. There-fore the health and safety of the public were not affected.

3. Cause:

Flow transmitter FT-2495 failed to function because the equalizing valve for the transmitter was not fully closed, thereby preventing proper operation of the transmitter.

4. Immediate Corrective Action:

The immediate corrective action was to place channel 4 high steam flow and channel 4 steam flow greater than feed flow Bi-stable switches to the trip mode.

5. Subsequent Corrective Action:

The subsequent corrective action was to fully close the equalizing valve for the transmitter.

6. Actions Taken to Prevent Recurrence:

No additional actions were deemed necessary.
