Loss of Audible Fire Alarm CONDITIONS PRIOR TO OCCURRENCE:

Mode 5 - Rx Power 0% - Unit Load 0 MW DESCRIPTION OF OCCURRENCE:

While performing routine testing of the Fire Detection System IAW the Salem Generating Station Fire Fighting Manual, it was discovered that no audible alarm signals were being generated. Action State-ment 3.3.3.6.a was entered at 1256.hours.

This occurrence constituted operation in a degraded mode IAW Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment Failure - A limit switch in the No. 14 Decoder in the Tone Generating System was binding and prevented th.e decoder from completing the sequential action necessary to generate the audible alarm. The limit switch had drifted out of adjustment.

ANALYSIS OF OCCURRENCE:

Technical Specification 3~3.3.6.a requires that a fire watch patrol be established to inspect the zone(s) with the inoperable instrument(s) at least once per hour.

CORRECTIVE ACTION:

The 'decoder and its limit switch were readjusted, cycled several times and tested satisfactory. The Action Statement was terminated at 1526 hours0.0177 days <br />0.424 hours <br />0.00252 weeks <br />5.80643e-4 months <br />.
