No. 2 Fire Pump Inoperable CONDITIONS PRIOR TO OCCURRENCE Unit 1 - Mode 5 - Rx Power 0% - Unit Load 0 MW Unit 2 - Mode 5 - Rx Power 0% - Unit Load 0 MW DESCRIPTION OF OCCURRENCE:

On April 2, 1981, the No. 2 Fire Pump was declared inoperable because of a leaking Fuel Oil Pressure Gage Supply Hose. Action Statement 3.7.10.la, entered at 1400 hours0.0162 days <br />0.389 hours <br />0.00231 weeks <br />5.327e-4 months <br />, requires that with one pump and/or one water supply inoperable, restore the inoperable equipment to operable status within seven days or, in lieu of any other report required by Specification 6.9.1, prepare and submit to the Commission

.pursuant to Specification 6.9.2 within the next 30 days outlining the plans and procedures to be used to provide for the loss of redundancy in this system.

This occurrence constituted operation in a degraded mode IAW Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment Failure - Investigation revealed a small hole in the Fuel Oil Pressure Gage Supply Hose.

ANALYSIS OF OCCURRENCE:

The four foot long Fuel Oil Pressure Gage Supply Hose connects the Fuel Oil Injector Manifold to a pressure gage which supplies local indication of the fuel oil pressure. Examination of the hose suggests that the leak was the long term effect of the hose chafing against the m~nifold as the pump was operated.

CORRECTIVE ACTION:

Pending the arrival of a replacement hose from the vendor, the pressure gage was isolated from the manifold by installing a cap on the manifold hose connection. The Action Statement was terminated at 2030 hours0.0235 days <br />0.564 hours <br />0.00336 weeks <br />7.72415e-4 months <br /> on April 2, 1981.
