Rod Position Indicator Inoperative (2Dl)

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 30% - Unit Load 340 MW DESCRIPTION OF OCCURRENCE:

During normal operation on February 6, 1981, the Rod Position Indicator for Rod 2Dl indicated that the rod had dropped. The RPI was declared inoperable and Action Statement 3.1.3.2.a was entered at 2320 hours0.0269 days <br />0.644 hours <br />0.00384 weeks <br />8.8276e-4 months <br />. The Action Statement requires that with a maximum of one rod position indicator channel per group inoperable, either determine the position of the non-indicating rod(s) indirectly by the movable incore detectors, at least once per eight hours, and immediately after any motion of the non-indicating rod which exceeds 24 steps in one direction since the last determination of the rods position, or reduce thermal power to less than 50%

of rated thermal power within eight hours.

This occurrence constituted operation in a degraded mode IAW Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The Amphenol screw connector on rod 2Dl position indicator cable was loosely connected at the reactor head connection.

ANALYSIS OF OCCURRENCE:

Inspection of the connector revealed no damage to the pins or the connector itself.

CORRECTIVE ACTION:.

The Amphenol screw connector was tightened securely. Upon calibration and a satisfactory surveillance test, the Action Statement was terminated at 0730 hours0.00845 days <br />0.203 hours <br />0.00121 weeks <br />2.77765e-4 months <br /> on February 7, 1981.
