RI I: JRH 50-389/80-08 Infraction: Sand-Cement Fill - Procedure Inadequate As required by Criterion V of Appendix B to 10 CFR 50, and as implemented by Florida Power 8 Light Company PSAR Section 17. 1A.5, "Activities affecting quality shall be prescribed by documented instructions, procedures or drawings....and shall be accomplished in accordance with these instructions, procedui es, or drawings." Paragraph TgR 5. 1 of the Topical guality Assurance Report states, "Activities affecting quality of nuclear safety related structures systems and components shall be prescribed by documented instructions, procedures or drawings of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures or drawings. These documents shall include appropriate quantitative criteria such as dimensions, tolerances and operating limits and qualitative criteria such as comparative work-manship samples to assure that the quality assurance activity has been satisfactorily accomplished.

Contrary to the above, sand-cement fill activities in the diesel generating building are not being controlled adequately in that procedure drawings and instructions are not clear as to whether the stated 1 to 20 ratio of cement to sand is by volume or weight, water and compactions requirements are not specific, amounts of ingredients in the mix are not documented, and test data verifying design requirements are being met are not available.

~Res onse: Florida Power a Light Company has evaluated the finding and submits the following response. During this inspection DCN 513.822 was issued to clarify the ratio of cement to sand and to provide instructions for placing the sand-cement fill. In addition, Construction guality Control sampled a mix with the same mix properties used at the Diesel Generator Building. Five 6 inch diameter by 12 inch high cylinder specimens were prepared. The mix was 20: 1 by volume. Each specimen was prepared by filling the cylinder mold in six inch lifts, sprinkling the surface lightly with water and tamping to approximate the compacting efforts applied in the field. The first sample specimen was tested at seven days and reached 1110 psi. The twenty-eight day strength cylinders averaged 1310 psi. Al']
