ATTACHMENTRESPONSETONOTICEOPVIOLATIONVIOLATIONATitleTenCodeofFederalRegulation(10CFR),Part50,AppendixB,CriterionV,requiresactivitiesaffectingqualitybeaccomplishedinaccordancewithprocedures.10CFR,Part50,AppendixB,CriterionXVII,requiressufficientrecordsbemaintainedtofurnishevidenceofactivitiesaffectingquality.AmericanNationalStandardsInstitute(ANSI)N45.2.9-1974,paragraph3.2.1specifiesthatQualityrecordsaretobelegible,completelyfilledoutandadequatelyidentifiable.TheprecedingrequirementsareimplementedbyFloridaPowerandLightProcedureQI17-PR/PSL-1,Revision14,"QualityAssuranceRecords",Paragraphs5.1.C,5.2.2and5.2.7whichrequirethatrecords,bothhardcopiesandmicrofilm,bereviewedforcompletenessandlegibility,andreadability.Contrarytotheabove,onNovember3,1989,activitiesaffectingqualityhadnotbeenaccomplishedinaccordancewithprocedures,whichresultedinincompleteandillegiblerecordsofactivitiesaffectingqualityinthatthefinalqualityrecords(microfilm)forsevenPlantWorkOrderpackagesoutoftenexaminedcontainedmissingdates,missingsignatures,illegibledata,andillegiblesignatures.RESPONSETOVIOLATIONA:1.FloridaPower&Lightconcurswiththeviolation.2.ThereasonfortheviolationispersonnelfailuretopayattentiontodetailuponcompletionandreviewofPlantWorkOrderdocumentation.3~FloridaPower&LighthasreviewedthespecificitemsaswellasadditionalPlantWorkOrdersandhasfoundnosafetyconcerns.Theseitemswereconsideredadministrativeinnatureand,assuch,specificcorrectiveactiontoobtainthemissingsignaturesanddatesisnotwarranted.4.FloridaPower&LighthaschangedtheapplicableproceduretoclarifywhichspacesandblocksarerequiredtobecompletedonthePlantWorkOrder.Inaddition,theQualityControl(QC)SupervisorhascounselledhispersonnelonensuringthattheproperdocumentationisobtainedpriortoQCapproval.>IEachmaintenancedisciplinehasbeeninstructedtoensurethePlantWorkOrderhasbeenproperlycompletedbeforesubmittingtoQCforreview.5.TheaboveactionhasbeencompletedasofFebruary12,199 ATTACHMENTPage2of2VIOLATIONB:10CFR50,AppendixB,CriterionVrequires,inpart,thatactivitiesaffectingqualityshallbeprescribedbydocumenteddrawingsofatypeappropriatetothecircumstanceandshallbeaccomplishedinaccordancewiththesedrawings.Contrarytotheabove,onOctober3,1989activitiesaffectingqualitywerenotinaccordancewithdrawingsinthat:1.Unit1ExcoreNeutronFluxPanelRY2680Bprintedcircuitboardprotectiveenclosurewasnotinstalled(missing)asrequiredbyGammaMetricsdrawing200646.2.PowersupplyassemblybottomprotectivecoversforUnit1ReactorProtectionSystemPanelsMB,MCandMDwerenotinstalledasrequiredbyEEControlsDrawingsD-10089.3~Two120VACInstrumentBus2MCDistributionPanelfuses,FU-3,requiredbycontrolwiringdiagramCWA-2998-B-327,sheets1009and1636,weremissing.ThefusesarepartofthepowermonitoringlogiccircuitfortheAuxiliaryFeedwaterActuationSystem.RESPONSETOVIOLATIONB:1.=-FloridaPower&Lightconcurswiththeviolation.2.Personnelfailedtofullyrestoreequipmentduringtestsandchecks.3.Immediatecorrectiveactionwastakentocorrectthespecificdeficienciesexceptthedustcoversinthereactorprotectiondrawers.TheabsenceofdustcoversonthereactorprotectionsystemhasbeenevaluatedbyFPLandfoundtobeanacceptableinterimplantconfiguration.NewcoversareonorderandexpectedtobeinstalledbyAugust1990.4~5.Supervisorypersonnelhavebeendirectedtoensureallrestorationworkiscompleteattheendofthejob.Inaddition,walkdownshavebeenconductedbysupervisorstoidentifyandcorrectsimilartypedeficiencies.Fullcompliancehasbeenachievedexceptforthecoversnotedabove,whichisexpectedtobecompletedbyAugust199 IPV"
