ATTACHMENT VIOLATION:

During the Nuclear Regulatory Commission (NRC) inspection conducted on December 6, 1987- January 2, 1988, a violation of NRC requirements was identified. The violation involved the failure to establish and/or implement procedures. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), the violation is listed below:

Unit 1 Technical Specif ication (TS) 6. 8. 1. a, states that written procedures shall be established, implemented and maintained covering the applicable procedures recommended in Appendix A of Regulatory Guide 1.33, Revision- 2, February 1978.

Regulatory Guide 1.33, Revision 2, February 1978, Appendix A, states in part, that procedures are required for startup, operation and shutdown of safety-related systems including electrical systems.

Contrary to the above, on December 21, 1987, procedure 1-0970020, operation of the 120V instrument AC system (Class lE), was not properly implemented in that. while

-attempting to place the 1MD instrument invertor back into service following preventive maintenance, the inverter's output breaker was not closed, thereby causing loss of 120V instrument AC and a subsequent

'eactor trip.

RESPONSE

1. Florida Power & Light Company concurs with the violation.
2. A cognitive personnel error, by a non-licensed operator not adequately following a Plant procedure, is the cause for the violation.
3. The 1MD inverter was restored to normal operation. The non-licensed operator involved was counselled on his error and his qualifications were reviewed.

4 ~ A procedure governing the control of infrequent operations on sensitive 'systems is under development to insure adequate expertise and planning is present during these infrequent evolutions. Non-licensed operator training is being assessed for effectiveness in the area of Electrical Plant Distribution.

5. The long term corrective action is scheduled to be completed by June 1, 1988.
