DUQUESNE LIGHT COMPANY Beaver Valley Power Station Unit No. 1 Reply to Notice of Violation Inspection 87-05 Letter dated March 31, 1987 VIOLATION (Severity Level IV; Supplement I)

Descri tion of Violation (87-05-02) 10 CFR 50, Appendix B, Criteria XIV, Inspection, Tests and Operating Status, requires that measures be established for indicating the operating status of systems such as by tagging valves to prevent inadvertent operation. OM Chapter 1.48.6B, Mechanical and Electrical Clearance Procedures, provides detailed instructions for controlling plant configuration that require equipment removal and restoration by the use of red danger tags to define the clearance boundary points and control room status boards to identify components out of normal system alignment.

Contrary to the above, the A degassifier was returned to service on February 13, 1987, without adhering to the requirements of OM 1.48.6B in that all clearance points were not returned to normal system alignment

prior to returning the degassifier

to service.

Specifically, various vent lines were left open that were not identified on the equipment clearance or control room status boards,

resulting in a minor unplanned radioactive gas release to the primary

~

auxiliary building. ~

Corrective Action Taken

1. A proper valve alignment was performed to fully isolate the degassifier. The degassifier has since been properly purged and returned to service.

Action Taken to Prevent Recurrence

1. The Operations Supervisor held discussions with all Operations personnel concerning this incident, the topics discussed were:

A. A chronological sequence of events for this incident.

B. Proper configuration control per requirements of OM Chapter 1.48.6B "Mechanical and Electrical Clearance Procedures",

and proper methods for returning to service systems that routinely process radioactive materials.

C. Good operating practices and'he use of proper attention to detail concerning complex evolutions of a sensitive nature.

2. An Operating Manual Change Notice was generated to eliminate

procedure deficiencies in Operating Manual Chapter 1.8.4.GG, "Purging Degassifiers", which contributed to this event.

3. Individuals directly involved in the evolution will be personally contacted and counselled concerning their involvement with the incident, specifically, their failure to adequately control the evolution.

Date on Which Full Com liance Will be Achieved Concerning Items 1 and 2 above, full compliance has been achieved at this time. Concerning Item 3 above, actions will be completed by May 31, 1987.
