LICENSED OPERATOR REFUSED RANDOM DRUG TEST

Dear Mr. Williams:

On November 8, 2005, you reported in Event Report 42128 that one of your NRC-licensed operators refused to take a random fitness-for-duty drug test. You stated that this individual had tendered his resignation on November 4, 2005, and it was to become effective November 11, 2005. However, upon his plant access being denied for refusing to take the random drug test, the individual subsequently resigned his position effective that same day, November 8, 2005.

In order to facilitate our review of this matter, we require additional relevant information beyond what you have reported to date. Therefore, we request that you provide answers to the questions listed in the enclosure and forward to us any other records or information you may have on this operators fitness for duty within 30 days after the date of this letter. Any personal, proprietary, or safeguards information in your response should be contained in a separate attachment and appropriately marked. An affidavit required by 10 CFR 2.390(b) must accompany your response, where applicable.

The NRC will evaluate the information in your reply to this letter to determine if further action is warranted pursuant to 10 CFR Parts 26 or 55. The information provided by you will be subject to the Privacy Act of 1974 and it will be maintained in the Privacy Act System of Records, NRC-16, Facility Operator Licensees Record Files (10 CFR Part 55).

ENCLOSURE Licensed Operator Fitness-for-Duty Questionnaire Grand Gulf Nuclear Station is requested to provide the following information concerning the fitness-for-duty occurrence of November 8, 2005 (Event Report 42128):

1. Name and job responsibilities of the licensed operator for the last 12 months.
2. A summary of the operators entire fitness-for-duty testing history. Please include the dates and times the operator was tested, the reasons for the tests (i.e., random, for cause, or followup), the results of the tests, and the dates that any tests were confirmed positive.
3. A detailed chronology (time line) associated with this event.
4. A detailed summary of your review of the event, including all relevant information developed in the course of your review (e.g., Condition Report, summaries of interviews, etc.). Include the reason(s) of the individuals earlier than planned resignation if known.
5. A list of actions taken or planned as a result of this event.
6. Any other relevant information that will facilitate the NRC review of this matter.
