Followup on Previous Inspection Findings (Closed) Violation (.458/8630-01):

Failure to Identify an Inoperable Fire Door and Establish the Required Fire Watch - The NRC inspector reviewed the licensee's long-term corrective action which consisted of revising STP-000-3001, " Daily Fire Door Position Check," to clarify the acceptance criteria for normally closed fire doors to include a check of latches and closing mechanisms.

During the area walkdown, the NRC inspector did not find any inoperable fire doors.

This violation is closed.

(Closed) Unresolved Item (458/8630-02):

This item was considered unresolved pending action by the licensee to identity acceptable and unacceptable fire door conditions.

The licensee identified and corrected a number of fire door conditions during the investigations associated with I

Violation 458/8630-01.

These were reported in the December 22, 1986, response to the violation and in Licensee Event Report (LER)86-066.

The NRC inspector did not observe any degradation of barrier rating during the plant area walkdown.

Procedural guidance was provided to control combustible material and reduce fire hazards.

Administrative procedures also provided for maintenance and surveillance on fire suppression, detection, and support equipment.

Personnel training, qualifications, and responsibilities were adequately provided.

Maintenance evolutions that significantly increase fire risk were properly controlled.

The NRC inspector toured accessible areas of the plant site for general area condition, work activities in progress, and visual condition of fire protection systems and equipment.

combustible liquid and gas usage were restricted or properly controlled in areas containing safety-related equipment and components.

Items checked included positions of selected valves, fire barrier condition, hose ctations, hose houses, halon system lineups, fire lockers, and fire extinguishers for type, location, and condition.

.There was no welding, cutting, or use of open flame ignition sources found in the areas toured.

Refueling outage preparations were underway.

Some maintenance work and surveillance testing was noted.

General housekeeping conditions were found to be very good.

Fire brigade equipment, including-emergency breathing apparatus, was found to be properly stored and maintained.

Fire protection systems and equipment installed for protection of safety-related areas were found to be functional and tested

in accordance with the requirements specified in the TS.

q The NRC inspector also reviewed fire brigade training and drill records.

The records were in order and confirmed that training and drills were being conducted at the specified intervals.

The last QA audit in the area of fire protection which served as the annual and triennial audit and is identified as Audit No. 87-03 I-PFPP of March 17-27, 1987, was reviewed by the NRC inspector.

This audit was comprehensive and in-depth.

System / equipment alterations, tests, surveillance, maintenance, records, and overall program procedures were addressed.

Discrepancies identified were formally presented to the affected organization.

Responses were tracked to closecut and actions j

taken were reviewed for adequacy.

There were no violations or deviations identified in this area of the t-inspection.

5.

Exit Interview An exit interview was conducted on September 25, 1987, with those personnel denoted in paragraph 1 of this report.

At this exit interview, the NRC inspector summarized the scope and findings of the inspection.
