Discrepancy between the FSAR and the Test and Startup Manual (TSPM):

FSAR Sections 14.2.4.1.1 and 14.2.4;].2 where changed to be consistant with the licensee's Test and Startup Manual.

Plant Administrative Procedur'e'Review:

The inspector reviewed the following administrative. procedures to determine the licensee's program for plant procedure preparation and general employee training:

Administrative Procedure (AP) 1.2. 1 - Plant Procedures. gqnual,Description AP 1.2.2 - Plant Procedure Preparation AP 1.2.3 - Use of Plant Procedures AP 1.2.4 - Plant Procedure Approval and,Revision AP 1.2.S - Plant Procedure Distributi.on and Control AP 1.8.1 - General Employee Training The inspector made the following comments regarding plant procedure preparation:

a)

A method needs to be developed to keep the training department informed of procedure and design changes.

This item was discussed with the licensee and agreement was reached to develop a means to keep the training department current with these changes.

b)

The issue of independent verification was raised by the inspector as part of the TMI action plan item I.C,6.

The licensee has been working on this item; however, action has not been formalized in the administrative procedures.

The inspector informed the licensee that this would remain an open item until the licensee has formalized a policy in this regard (81-04-02).

c)

The licensee includes a general gA indoctrination lecture for all employees; however, specific gA requirement implementation for job related activities is left to the supervisor.

The inspector reviewed the training program for the maintenance personnel as developed by the maintenance supervisor.

The maintenance training program was complete in that it gave specific training to each discipline (e.g. electrical, mechanical);

however, the program was not formalized in the maintenance training procedure.

The maintenance supervisor agreed to formalize this program and issue a change to the applicable Maintenance Traning Procedure (81-04-03).

Review of Diesel Generator Heatin and'Ventilation'Prep er'ational'Test PT88.0 5.

The inspector reviewed, the test procedure to verify commitments of the FSAR and Regulatory Guide 1.68 have been honored.,

The inspector did not have any specific comments to this procedure, but during t'e course of the review, several areas of concern were identified; a)

The licensee is committed during the preoperational test program to trial-test plant operating and emergency procedures.

To ensure the applicable operating and'emergency procedures are ready to support each test, the licensee agreed to add a

r'equirement to the TSPM for the Test Working Group.(TNG) to review the status of operating and emergency pr'ocedures issuance to support the applicable test during the j'HG,meeting two weeks prior to performance of the test.

b)

On occasion, the licensee may use contractors to perform certain tests.

The control of these contractors has not.been specified in the TSPM.

The licensee agreed to add a requirement to the. TSPQ to require TWG approval of. the contractor's. test pr'ocedure and test results (81-04-05).

c)

The licensee's O.C. program allows startup test engineers to perform g.C. inspections.

The licensee agreed to deyelop-'a program whereby certification is proyided for each discipline the test engineer is qualified to perform g.C. inspections (e.g.

electrical, mechanical, instrumentation, and control). '1-04-06}

Exit Interview The inspector met with the licensee representatives (denoted in paragraph 1) at the conclusion of the inspection on,Nautch 5, ]981.
