nspec on Branch Engineering and Technical Inspection Division SUMMARY Inspection on August 18-21, 1981 Areas Inspected This routine, unannounced inspection involved 28 inspector-hours on site in the areas of QA inspection of electrical / instrumentation and a licensee identified construction deficiency.

Results Of the areas inspected, no violations or deviations were identified.

Coastruction Deficiency Report (92700)

(Closed) LII 369/80-10-02, " Environmental Qualification of Equipment." On Novenber 8, 1979 Duke Power Company notified IE Region II of 50.55(e) item concerning the above. This item is closed based on the fact that this will be handled in the review required by NUREG 0583.

Field Drawings and Work Procedures The inspector randomly selected the following identified field design drawings and procedures to ascertain whether the most recent revisions of drawings are used to perform work and whether the most recent revisions of construction specifications and work procedures are in agreement with the SAR.

Field Inspection The inspector selected safety-related cables 2*EPE 510, 2*EPE 508, 2*EPE 515, 2*EPE 504, 2*EPE 507, 2*EPE 519, 2*EPE 506, 2*EPE 516, 2*EPE 518, 2*EPE 512, 2*SM 586, 2*SM 573, 2*SM 583, 2*SM 582, 2*SM 579, 2*SM 578, 2*SM 581, 2^SM 574, 2*SM 584, 2*SM 576, 2*SM 574 and 2*SM 580 for examination.

The inspector made a detailed inspection of the as-built cable instal-lations to determine whether equipment or systems were installed as described by field drawings and construction specifications.

While performing the #ield inspection verifying cable routing for cable numbers 2*SM f~a 2*EPE 512, and 2*EPE 518 the inspector observed that some of the saf ety-related stickers were missing from the cable trays.

The inspector questioned the licensee concerning the installation of the safety-related stickers and junction points on cable raceways. The licensee had identified this concern in the last QA surveillance report dated June 26, 1981.

The licensee issued nonconformance item report (NCI) 13459.

The licensee's corrective action requires that safety-related stickers pulled off or loose be reapplied using a loctite glue or be painted.

The inspector questioned disposition of the NCI by construction without it being approved by desigr. The NCI references specification MCS-1390.01-00-61 as the specification for identifying

safety related cable trays, conduits and wireways but the appropriate specification is MCS-1390.01-00-36. The above mentioned specification states in part, "all safety related cable trays, conduits, wireways, etc., associated with the above systems are marked with these colors".

This specification does not describe what construction methods are acceptable for identifying cable raceways. Th's item is unresolved and is identified as 359/82-27-01 and 370/81-15-04, Methods used to iden-tify safety-related cable tray, electrays and conduits.

The inspector also talked to electricians and foreman associated with i

safety-related cable installations to determine whether their level of knowledge was adequate to provide the required quality of workmanship.

Field Design Changes Field design changes are initiated with a Variation Notice (VN) which must be approved by design. The site can receive initial approval from-design by telecon and proceed with the work. A copy of the VN will be transmitted to corporate design for final approval where corporate design may take exception to the recommended disposition. Design will revise drawings to incorporate the variation and issue new drawings to the site.

The inspector selected the following VN's for review to determine whether the reasons for the changes were adequate; changes did not appear to compromise the original design intent; changes were reviewed and approved by other than originator and review did consider impact on overall design; and design drawings are revised to incorporate changes.

"NRC Reporting Requirements" These documents were reviewed to determine whether procedures, inclu-ding frequency and timing of inspections are adequate to properly control the work, and that inspection procedures and reference docu-

-ents are adequately detailed to instruct the QC inspector on exactly what he should be looking for when performing inspections.

The RII inspector discussed inspection requirements w;th several QC inspectors to determine if they were knowledgeable of the procedures, if they considered the number of inspectors acceptable to do the work and whether management responds correctly to negative findings.

No violations or deviations were identified in this area.

The inspector reviewed nonconforming items reports 13600, 13588, 13556 and 13459. Corrective action was examined to verify that the actions taken corrected the items, determined the cause of the deficiency, considered the reportability to NRC and instituted effective action to prevent recurrence. One unresolved item was identified in this area in paragraph 6.b.

e.

Materials Certification The inspector verified that cable reels 00420, 00354 and 00498 had been certified acceptable for construction.

Within the areas examined, one unresolved item 369/81-27-01 and 370/81-15-01 was identified.
