Exit Interview The inspection scope and findings were summarized on September 12, 1980 with those persons indicated in Paragraph I above.

Licensee Action on Previous Inspection Findings (Closed) Infraction 338/'9-40-01, Failure to Follow PT Procedure.
VEPCO's letter of response dated December 20, 1979 has been reviewed and determined to be acceptable by Region II.
The inspector held discussions with the Mechanical Foreman - NDT and examined the corrective actions as stated in the letter of response. The inspector concluded that VEPCO had determined the full extent of the subject noncompliance, performed the necessa ry survey and followup actions to correct the present conditions and developed the neccesary corrective actions to preclude recurrence of similar circum-stances. The corrective actions identified in the letter of response have been implemented.

t (Closed) Unresolved Item 338/79-43-01, ET Instrument Calibration Drift-Tube Reexamination. Provisions for reexamination of steam generator tubes where calibration drift was identified during eddy current examination has been incorporated into the procedure by revision.

(Closed) Unresolved Item 338/79-43-02, Description of Partial UT Examination of Welds. A review of ISI records generated subsequent to the inspector's comments showed that the reasons for partial ISI weld examinations were delineated and weld sections omitted were fully described.
(0 pen) Infraction 338/79-47-01, Deposited Weld Metal Thickness - Weld Proce-dures and Welder Qualification. VEPCO's letter of response dated January 15, 1980 has been reviewed and determined to be acceptable by Region II. The inspector held discussions with the station manager and examined the correc-tive actions as stated in the letter of response. The inspector concluded that VEPCO had not determined the full extent of the subj e t noncompliance and had not perforted the necessary survey and follow-up actions to correct the present conditions. Hence this matter remains open until the licensee has implemented the appropriate corrective actions and full compliance has been achieved. Further discussion on this matter appears in paragraph 5.b of this report.

5.
Independent Inspection Effort Visual Examination Procedure - Welding.
a.
The inspector held discussions with co;71izant QC and NDT personnel in order to ascertain whether the North Anna and VEPC0 program for inspec-i ting safety related welds upon completion was consistent with Code and
,
NRC requirements.
This discussion and subsequent procedure review revealed that training and certification of QC inspectors is handled by a VEPCO level III examiner. Candidates are certified by examination.
Completed welds are inspected by individuals certified as QC inspectors (general qualification) and/or visual (welding) inspectors. Acceptance /-
rejection criteria for visual inspections cre based on the code applica-ble to a particullar weld, e.g.
A.W.c 5-1 1, ANSI B31.1, or ANSI B31.7.
In response to the inspectm a raquut to review the visual weld inspection procedure used to Or y., t this activity, the licensee representative stated that suct p M are did not exist at this
.n time; this was in agreement with the sinement made earlier in the week during an inspection of the Surry Staticn. The inspector stated that since activities affecting quality must be accomplished by documented instructions / procedures which contain appropriate acceptance /
rejection criteria it would be incumbent upon them to generate such a procedure for use in the field.

In that this problem appears to be common to the North Anna and Surry

stations, i.e.
see IE:II Report 50-280/80-35, noncompliance 50-280/
80-35-01, the licensee agreed to look further into this matter and determine whether a procedure applicable to both stations could be generated.
The inspector stated that this matter woald be identified as unresolved item 50-338/80-33-02, Visual Weld Inspection Procedure,

until RII has the opportunity to review the steps taken to correct

b.
Welding Procedures and Welder Performance Qualifications As a followup to VEPCO's response to notice of violation 50-338/
~
79-47-01, dated January 15, 1980, the inspectot interviewed cognizant i
station personnel and reviewed certain welding procedure qualifications in order to ascertain whether the corrective actions stated in the aforementioned letter had been achieved by the stated date of March 15, 1980. Within these areas the inspector noted that the proposed correc-tive action had not been achieved as none of the combination process procedures had been revised.
The inspector discussed this matter with the licensee who agreed to submit a supplemental response to the noncompliance to IE:II by September 19, 1980.
Following the close of this inspection, on September 23, 1980 the licensee was contacted by telephone and informed that failure to achieve corrective action by the March 15,.1980 date stipulated in VEPCO's letter of response S/N 1171/121979 dated January 15, 1980 was
in noncompliance, infraction category, with Criterion XVI of Appendix
'
d to 10 CFR 50. This finding was identified as item 50-338/80-33-01, Failure to Take Corrective Action.
