Exit Interview The inspection scope and findings were summarized on November 16, 1979, with those persons indicated in Paragraph 1 above.

Unresolved Items Unresolved items were not identified during this inspection.

Licensee Event Report Followup On November 8,1979, the licensee informed NRC of a reportable occurrence concerning design of certain ventilation dampers installed in the Control Room Ventilation System and the Safeguards Building Exhaust Ventilation System. Since then, on November 14 and again on November 16, the licensee forwarded supplemental 24-hour reports concerning other systems found to have similar design errors.

In each case involved, a system component which functions during a Containment Depressurization Actuation (CDA) or, in one instance, Safety Injection (SI), was found to have its operation or operability altered when the CDA Reset or SI Reset pushbuttons are used.

The exact alteration to the operability of each of the seven systems varies slightly, but in each case indicates inconsistency with FSAR comment 7.4.

The inspector reviewed the circuitry involved in three of these systems with licensee staff.

In each case, the individual motor, valve, or damper control circuit is directly affected by the CDA actuate signal, which once removed by depressing CDA Reset, either returns the control system to normal configuration or prevents the circuit from restarting should a blackout subsequently occur. The licensee shall be submitting a 14-day LER, number 79-141, to report all of the design errors.

Correction /resolu-tion of these deficiencies shall be followed as an open item (338/79-46-01 and 339/79-55-01).

.Previously Identified Items a.

Unit 2: FSAR To As-Built Concerns The inspector reviewed licensees drawings, FSAR and applicable Engineering and Design Change Requests (E&DCRs) to followup on deficiencies identified in IE Reports 339/78-35, 79-13, 79-15, 79-22, and 79-38.

(Closed) Open Item (339/78-35-02):

Casing Cooling System inter-face not shown on print 12050-FM-91A-7.

Print 12050-FM-91A-10 now correctly shows where the Casing Cooling System ties into the Recirculation Spray System.

2)

(Closed) Open Item (339/79-13-01): Two valves on print 12050-FM-96A-8 not installed. E&DCR 24568F-2 documented revision of the SI system to delete these valves.

Print 12050-FM-96A-9 correctly omitted valves 2-SI-264 and 2-SI-266.

(Closed) Open Item (339/79-13-02): Two valves on print 12050-FM-96B-7 not installed. E&DCRs P-1353K-2 and P-1353L-2 documented this revision to the SI system.

Print 12050-FM-96B-8 now correctly omitted valves 2-SI-242 and 2-SI-244.

(Closed) Open Item (339/79-13-03): Three small local accumulator sample valves not shown on print 12050-FM-96B-7. These valves are small " Hoke" valves which, although not shown on the 3/4 inch sample lines, are indicated by the term " Local Sample".

Isolation and drain valves on print 12050-FM-96B-7 incorrectly positioned E&DCRs 25341A, 25341A-1 and 25341B documented alterations to the isolation and drain valve positions on the hot leg and cold leg injection lines.

Print 12050-FM-96B-8 correctly depicts placement of these valves now.

Incorrect FSAR drawing of the Auxiliary Feedwater Pump 2-FW-P-2 discharge piping.

FSAR figure 10.4.3-4 still incorrectly shows the pump discharge line to 'A'

Steam Generator taping off the 6-inch discharge line before the line splits to feed the two common auxiliary feedwater pump dis-charge headers.

(Closed) Open Item (339/79-15-05): Drain valve not shown on print 12050-FM-74A-10 or in the FSAR.

Print 12050-FM-74A-11 now properly depicts drain valve 2 -FW-190.

An earlier valve tag error was also corrected so that valve 2-FW-189 is correctly identified. The FSAR is not required to display piping drain valves.

(0 pen) Open Item (339/79-22-02): FSAR incorrectly depicts drains and isolation valves on SI lines. Paragraph 6.a(5) above discusses correction of print 12050-FM-96B on the same group of isolation and drain valves in the SI system. This correction is not yet reflected in FSAR figure 6.3-2.

Two of the three errors concerned vent or drain valves, which are not required in FSAR figures. However, the third error, which still exists on figure 6.3-1, is the incorrect

placement of valve MOV-1885A (2885A for Unit 2 on a Unit I and 2 drawing).

(0 pen) Open Item (339/79-22-06): Valve locations in FSAR figure 5.5.4-1 in error. Two of the three errors again concerned vents and drains. The third error concerns placement of PIC 2602 (1602 on FSAR figure 5.5.4-1 for Units 1 and 2) which is still in error compared to 12050-FM-94A-7.

(Closed) Open Item (339/79-38-01): Three vent / drain taps not shown on print for Pressurizer Relief Tank level indicator. The inspector reviewed prints 11715-FK-5F-7 and 11715-FK-FK-5D-7, where the vent taps are shown as installed. These taps are apparently instrument test corrections and maintenance taps which are not shown on FM drawings.

The five remaining open items were confirmed by the inspector using FSAR figures updated through Amendment 67 of July 31, 1979. These errors were detailed to licensee staff at the plant and separately, subsequent to the inspection, to licensee staff at the corporate office. No apparent items of noncompliance or deviations were identified.

(Closed) Open Item (338/79-30-03): Supplemental report to LER 79-55. As discussed in IE Report 338/79-41, licensee management had reviewed the Maintenance Reports (MRs) of March 30, 1979, and had resolved the issue as being nonreportable. The inspector

-4-reviewed licensee staff documentation of this review, including Deviation Reports79-481 dated September 27 and 79-493 dated October 3 and internal correspondence dated October 8 which explained the errors in the fifteen MRs documenting work on Individual Rod Position Indication (IRPI) circuitry on March 30.

The inspector also discussed the events with operations personnel and confirmed that during mode 2 operations, none of the IRPIs exceeded 12 steps differential from rod bank position.

Licensee management was informed that a supplemental LER was not deemed necessary in light of these facts.

(0 pen) Open Item (338/78-32-02):

Improvements to Containment Chill Water System. The inspector discussed design changes to the containment cooling systems which are still underway to maintain average containment air temperature with technical Specification 3.6.1.5 limits. The inspector shall verify these measures completed 'n future inspections.
