(Open) IE Bulletin Number 79-02, Pipe support base plate designs using concrete expansion anchor bolts (Unit 1)
This followup inspection concerned licensee activities related to the actions outlined in IE Region II letter of September 7, 197 The VEPCO procedure governing the expanded inspection program, Special Test Procedure
.SI-75, "Baseplate Visual Inspection and Anchor Bolt Test Program" was reviewe A sample of the marked up isometrics used for the gross defects, walkdown was examined for the types and extent of deficiencies identifie The inspector witnessed inspection and testing of four anchors by one crew and reviewed completed data sheets and interviewed inspection team members of another cre Inspection crews consisted of an engineer, a QC inspector and mechanic The work effort appeared to be thorough and accurat At the end of this inspection the licensee had inspected 72 anchor installa-tions, 12 of which required further effo.rt to complete the inspection or test (shell against plate, etc.).
Two anchors had spun in the hole and the test torque could not be obtaine The licensee has selected four safety injection and three reactor coolant lines with a total of 27 support~ for the small bore piping design verifica-tion specified in Item 1 of the IE:RII September 7, 1979 letter. Per the licensee these seven problems together with the analyses peformed previously under the seismic reanalysis show cause order comprise all of the safe shutdown piping inside containmen Baseplate flexibility will be included in these analyses as well as the analyses of as-built baseplate discrepancies noted in the original sample progra No items of noncompliance or deviations were identifie.
(Open) IE Bulletin Number 79-14, Seismic Analysis for As-Built Safety-Related Piping Systems (Units 1 and 2)
VEPCO QC and Stone and Webster (S&W) site personnel were interviewed con-cerning the licensee's program for piping and support as-built verificatio After the initial walkdown of a piping run marking up the isometri~ S&W Field QC representatives perform another walkdown verifying the accuracy of the marked up isometri VEPCO QC is witnessing a sample of the S&W QC verification and is independently verifying another sampl The inspector examined as-buil~ sketches for pipe supports and marked up isometric The results of the VEPCO QC walkdown inspection of supports as-built by S&W for the seismic reanalysis show cause order were reviewed and discussed with involved personne The inspector witnessed S&W QC verification of a portion of CVCS system piping shown on isometric 110 A crew preparing support as-built sketches was also observed and interviewe VEPCO indicated that supports evaluated during the seismic reanalysis Show Cause Order as having design loads less than or equal to original design
