Unresolved Items Unresolved items were not identified during this inspectio Steam Generator Replacement/Repair Program The licensee program for repair/replacement of steam generators was reviewe This included review of numerous sub-parts of the single design change (DC-77-45) called Engineering Task Assignments (ETA), a

-2-tour of facilities involved, interviews with numerous management per-sonnel and a briefing by the Director of Production Technical Suppor VEPCO is managing the complete steam generator replacement project (SGRP).

VEPCO has obtained personnel for engineering and design support from Stone and Webster Engineering Corporation (S&W).

The S&W engineers are under direct supervision of VEPC Daniel Construction Company (DCC) will be the major site contractor. VEPCO will perform overall directional control of site contractor activitie.

Review of QA Program Inspector reviewed the QA program to ascertain whether QA plans, instruc-tions, and procedures have been established to control the safety related activities and that they conform to the QA program as described in the Safety Analysis Report, VEPCO's submittal for the Steam Generator Replace-ment Project and the NRC Safety Evaluation Repor The following specifications, contracts, QA Manuals, and documents were reviewed by the inspector: SGRP - Surry Power Station QA Manual - Supplement Safety Evaluation by the Office of Nuclear Reactor Regulation dated December 15, 1978 VEPCO/DCC Contract dated May 15, 1978 DCC QA Manual Specification and Purchase Oider (Williams Crane and Rigging, Inc.)

for rigging, handling and hauling of steam generator lower section.

Chicago Bridge and Iron Company Nuclear QA Manual for repairs and alterations to ASME code stamped component.

Williams Crane and Rigging procedure for handling of new steam generator lower shells at barge slip and in temporary storag VEPCO, in addition to overall program control, will review and approve all contractor QA programs; perform procurement functions including site receiving, inspection, storage and issue and provide assurance of VEPCO/

Contractor QA/QC program implementation through surveillance and audit function The inspector held discussions with responsible VEPCO/S&W/DCC Engineers, QA/QC and craft personnel, reviewed controlling specifications, QA Manuals, procedures, and observed that facilities are provided for the support of the requirements of the above document The above identified documents

-3-confirm that VEPCO and/or their agents have established QA/QC plans and procedures to assure that the SGRP work will be accomplished in accordance with QA program requirements. The following areas were examined: Organizational structure and QA personnel Audits Quality Requirements Design control Work and Quality Inspection Procedures Control of material Control of process Corrective action Document control 1 Test control and control of test equipment 1 Quality records During the inspection and at the exit interview, considerable discussion was held with VEPCO management and QA/QC personnel regarding the following areas: The VEPCO Nuclear Power Station QA Manual Supplement which requires further clarification to more clearly define non-specific require-ments that are identified in the manual supplements. These relate to written notification requirements of significant conditions, audit, control, record retention and maintenance, nonconformance report control, corrective action contro.

Present manning of the VEPCO Resident QC Group which appears to be inadequate if work were to begin immediately, it appears that some augmentation will be required before work begin.

Training and qualification of VEPCO Resident QC Section personnel to be accomplished prior to participating in active QC Section function VEPCO personnel acknowledged the areas of concern and coauni tted to review of the program areas and make necessary change This area will receive further inspection in the near future (280/79-6-01; 281/79-6-01).
