Design Change and Modification Program The purpose 'of'this inspection was to assess the effectiveness of licensee's

activities 'to support' modifications and testing planned for the upcoming j

outage.

A number of design changes and modifications are planned for execution / installation during the forthcoming' refueling outage..The

!

' inspector randomly selected 10 modification packages for review. -These

-

packages are-identified in Attachment A.

Specifically, the inspector reviewed'the post. modification test specifications for adequacy and compliance with regulatory requirements.

It was noted that seven packages requi. red functional component tests.

Test procedures were not available

'for the three remaining modification packages.

The inspector identified the generic procedures to be used for the functional component tests. They are listed with the applicable work packages in i

Attachment A.

These generic procedures were used routinely for testing after maintenance or repair.

The system test procedures for IEC-2173,

'1890,land 2232, were not available in (PORC) approved form at the time of this infpection.

In.the review of modification package IEC-2202 it was noted that the torque switch settings for the new Jamesbury valves (butterfly valves) of the emergency service water system will not be supplied by the vendor.

For that reason, the as found values will be used for torque switch setting.

However, the licensee did not establish the engineering basis for using the as found values.

This item remains unresolved pending licensee action to establish the engineering basis for the use of as-found values for the torque switch settings (Unresolved Item 87-27-01).

No violations or deviation.s were observed by the inspector during this review.

3.

Independent Inspection Effort The licensee recently installed Pulsation dampeners on the suction and discharge of the Charging Pump (reciprocating pump) of Salem-2.

Vibration data had been obtained at various points in the system, before and after the installation of the pulsation chambers.

These data showed a significant reduction in the pulsation pressure ripple typical for reciprocating pumps.

Piping fatigue failures had been experienced at both units as a result of the induced vibration in the piping.

Considering these favorable test results it was decided to install the same pulsation dampeners on the suction and discharge of the Charging Pump of Salem-1. The pulsation dampener for the discharge side will be installed during the forthcoming refueling outage.

If there is time available the suction side will be done as well.

The inspector noted that the test specifications for the Salem-1 instal-lations (IEC-2065) does not include a post modification vibration test as l

was done for Salem-2. The licensee decided not to include the test re-l quirement in the test specification based on the successful installation I

and testing of dampeners at Salem-2.

Upon identification of this concern

by the inspector a licensee representative informed the inspector that this vibration test will be done after completion of the installation and the test specification will be revised to include this requirement.

No violations or deviations were observed by the inspector.

qA/QC Interface All of the test specifications in the modification packages listed in Attachment A were reviewed for QA interface requirements.

In all cases appropriate holdpoints and acceptance criteria were provided.- In the case of the installation specifications holdpoints and~

No violations or deviations were observed by the inspector.

Plant Tours The inspector made several tours of the plant: including the control room, switchgear rooms, auxiliary building, service building, turbine building,_

and safeguards areas to observe work in progress, housekeeping and

cleanliness.

No unacceptable conditions were noted.

Unresolved Items Unresolved items are matters'about which more information is required in Jorder to determine whether they are acceptable, an item of noncompliance,

or a deviation. Two unresolved-items were identified, as discussed in section 2 of this report.

Exit Interview At the conclusion of 'the site inspection, on September 25, 1987, an exit interview was conducted with the licensee's senior site representatives (denoted in Section 1) to discuss the findings.
