EXECUTIVE SUMMARY Monticello Nuclear Generating Station, Unit 1 NRC Inspection Report 50-263/98006 This inspection included a review of the implementation of the inservice inspection program for the 1998 Monticello refueling outage 18. The report covers on-site inspection by one regional inspecto Maintenance

Engineering involvement in the oversight of inservice inspection activities and jet pump hold down beam replacement was excellent. (Section M1.1)

. Overall, the inservice inspection activities were conducted in accordance with appropriate procedures and the overall inservice inspection program appeared technically sound and effective. (M1.2)

Nondestructive examinations were performed in accordance with applicable procedures by qualified personnel. (Section M5)

Corrective action to revise the inservice inspection plan to include the inspection of all reactor shell welds was implemented and effective. (Section M8)

Report Details M1 Conduct of Maintenance M1.1 Inservice insoection (ISI) Insoection Scoce (73753. 73755)

Inspectors observed ISI examinations and reviewed ISI examination data packages to assure appropriate examination was performed and data was recorded as required by the American Society of Mechanical Engineers (ASME) Boiler and Pressure Vessel Cod b. Observations and Findings The inspector observed work, radiation protection practices, reviewed ISI procedures, personnel certifications and reviewed data associated with the following activities:

  • Ultrasonic (UT) inspection of jet pump hold down beams (JPHDB) (beams 11-20)

e U T calibration and testing on residual heat removal suction piping (weld 4, 1S0-1S1-97003-B)

  • UT of core spray piping e UT calibration and inspection of jet pump number 16, mixer (MX) section welds MX2 and MX4 e Visual examination of steam dryer support bracket 215 Vendor personnel performed ISI to the ASME Code,Section XI,1989 Edition. Licensee activities observed were performed in accordance with applicable ASME Code requirement All applicable ISI procedures were approved by the Authorized Nuclear Inservice inspector (ANil) and were reviewed by the NRC inspector. The ISI procedures were found to be acceptable and in accordance with ASME Code,Section V,1989 Editio l During examinations of all 20 JPHDBs using UT, an indication was found on beam No.10. Good safety focus was demonstrated in meetings held to discuss the detected crack. The beam was subsequently replace I i

i Good contractor oversight was noted during the preparation for the UT of a jet pump riser while the fixture for the lower core spray UT was being removed. Both operations created confusion on the communications headsets. The cognizant engineer i immediately recognized the communications problem and took prompt action to suspend one operation until the other was completed to reduce the amount of confusion on the headset Conclusions The examination data was found to be in accordance with the applicable ISI procedures and ASME Code requirements. The inspector reviewed and evaluated the licensee's third ten-year interval ISI program plan for compliance with technical specifications, ASME Code and NRC requirement Additionally, the inspector reviewed ISI procedures used during the refueling outag Observations and Findinas The ISI program was conducted in accordance with ASME Section XI,1989 Edition. All ISI procedures reviewed were found to be in accordance with ASME Code,Section V and XI,1989 Edition requirement Conclusions

, Observations and Findings Qualification of personnel performing NDE work was verified. NDE personnel were knowledgeable of procedural requirements and proficient in the performance of ND Personnel performing NDE were found to have proper qualifications which had been reviewed and accepted by the licensee staff and the ANi Conclusions NDE was performed in accordance with applicable procedures by qualified NDE personne M8 Miscellaneous Maintenance issues M8.1 (Closed) Deviation (DEV) 50-263/96006-01: Failure to include the applicable Edition and Addenda of the ASME Code in ISI plan.

I Augmented examination of the reactor vessel required by 10 CFR 50.55a(g)(6)(ii)(A)(2)

specifies ASME Section XI,1989 Edition Table IWB-2500 (Category B-A) which requires the inspection of all reactor shell welds. However, the licensee included in the third ten-year ISI interval the 1986 edition requirements of Table IWB-2500-1 which required inspection of only one reactor vessel shell weld. The licensee's corrective action was to revise the inspection plan to schedule the inspection of all reactor shell weld V. Management Meetings
