NIAGARAMOHAWKPOWERCORPORATION.)eII'W,NIAGARAIIMOHAWKNMP-18422THOMASE.I.EMPGESVCEPRESOENT~OENEMTON300ERICSOULEVARDWCSTSYRACUSE,N.Y.l3202July31,1986Dr.ThomasE.MurleyRegionalAdministratorUnitedStatesNuclearRegulatoryCommission631ParkAvenueKingofPrussia,Pennsylv'ania19406,Subject:ResponsetoInspectionReportNo.50-220/86-10Sir:ThisletterisinresponsetotheNoticeofViolationenclosedasAppendixAtoInspectionReportNo.50-220/86-10.Theviolationconcernsplacingthein-coreHydrogenWaterChemistrySamplingLineinservicepriortoperformingtherequiredhydrostaticpressuretest.ThiseventhasbeenreportedinLicenseeEventReport86-20.Duetoanincorrectsequenceonthecontractorin'stallationprocedure,thehydrogenwaterchemistryincoresamplinglinewasplacedinservicepriortoperformingthehydrostaticleaktest,asrequiredbyASMESectionIII1980Edition,SubsectionNCwiththeSummer1982Addenda.Weldingwasperformedonthesamplelineafterthehydrostaticleaktestandsubsequently,apinholeleakdevelopedinthesamplelineoutsideofprimarycontainmengwhilethereactorwasatpowerandpressurized.Atthetimeofdiscoveryoftheleak,thereactorwasin,theprocessofbeingshutdownduetoanunrelatedproblem.Followingthecompletionofthereactorshutdown,acooldownwasinitiatedtolowerreactorcoolanttemperaturetofacilitatedisconnectingthesamplelinebetweenthereactorvesselandthesourceofleakage.AdrywellentrywasmadeandthesamplelinewasisolatedfromthereactorvesselbyremovingaSwagelokconnectorandcappingtheopenendsoftheline.Theprimarycontainmentpenetrationandthedisconnectedlinewerelocalleakratetestedtoverifypressureintegrity.eRepairswereeffectedontheleakingportionofthesamplelineonJuly9-11,1986duringaplantforcedoutage.Thedamagedsectionofsamplelinewasremovedandreplacedwithnewpipe.Toensurethatallweldingonthelinewascompletedpriortothefinalhydrostaticleaktest,additionaladministrativeholdpointswereincorporatedintothecontractor'srepairprocedure.Inaddition,anewhydrostatictestprocedurewaswrittenwhichincludedverificationofthecompletionofallweldingasaprerequisiteforcommencingthetest.TherepairedsamplelinewassuccessfullypressuretestedonJulyll,1986.Followingthesuccessfulcompletionofthe8608080140S6073i!PDRADOCN,05000220'Poa il2~U5'Zlj'gpgg+Pe""'">>-naggyCh Page2NMP-18422hydrostaticleaktestonthesamplelineandthelocalleakratetestonthepenetration,thesamplelinewasreconnectedtothereactorvesselbyreplacingthepreviouslyremovedSwagelokconnector.TheconnectorwasinspectedforleakageduringthenextreactorstartuponJuly14,1986withthereactorpressurizedat900psi.Noleakagewasnoted.Thecauseoftheviolationwasaninadequatefieldinstallationprocedurewhichallowedthesamplelinetobeplacedinservicewithoutthenecessaryhydrostaticleaktestbeingcompleted.Topreventrecurrenceofthisevent,therequirementsandspecificationsoftheASMEcoderegardinghydrostaticleaktestingwerereviewedwiththeplantengineerresponsibleforthismodificationandwillbereviewedwithplantengineersandQApersonnelresponsibleforsystempre-operationalandhydrostatictesting.Xnaddition,NMPCdesignengineeringpersonnelwillnowspecifythecoderequirementsforhydrostatictesting,includingthetestsequenceandboundaries.Verytrulyyours,8~~ThomasE.LempgesVicePresidentNuclearGenerationTEL/meed ldI"
