Operating Procedure OP-202, Plant Heatup, step 5.5.14, directs that the Reactor  !

Protection System (RPS) channels be removed frun shutdown bypass. h caution- 8 note associated with this step states that if at least one main feedwater punp is not latched prior to removing the RPS channels fran shutdown bypass, an emergency feedwater actuation may m,w. ,

Contrary to the above, on January 15, 1989, while performing a plant heatup, at- .'

least one main feedwater punp did not remain latched while -the RPS channels were removed fran shutdown bypass which resulted in an inadvertent actuation of-the emergency feedwater system.  :

This is a Severity Isvel IV Violation (Supplement I).

This violation was l reported in accordance with 10CFR50.73 as IER 89-03. i APPARENT CPOSE OF VIOIATIW The cause of the event was personnel error. The operator failed to recall the n4xlification which removed the auto bypass of EFIC, actuation below 20%. The l shift supervisor reviewed the procedure for the Main Feedwater Pump startup j and he also failed to recall the modification .which removed the auto bypass of EPIC actuation below 20%. The procedures did not provide . specific guidance on I the performance of these evolutions concurrently -and' are considered to be a  !

contributing factor.

(MRECTIVE ACTIGE The reglired actions of AP-450, Emergency Feedwater Actuation, were performed. j The cause of the EFIC actuation was immediately llentified and there were'no j!

indications of other abnormal conditions. The Main Feedwater Punp ,was relatched and the EFIC systen was restored to its wtomatic standby status. ,

DME OF FUIL CCMPLIANT Full otmpliance was achieved at 0438 on January 14,1989 when the EFIC system was restored.

ACTI(N TMQN 'IO HEVENT RBQJRRENCE Operations Management held a meeting for shift supervisors and assistant shift supervisors to diarnaa the inspector's concern involving the supervisory weakness durirg this event. 'Ihis event and associated corrective actions will be reviewed with all cont.rol board operators in order to enhance awareness and prevent recurrence.

OP-202, Plant Heatup, will be revised to instruct the operator to cmplete the applicable portion of OP-605, Feedwater System, before continuing,

'Ihe "CAUTICN" following Step 5.5.13 of OP-202 will be revised to indicate that an EFIC actuation will occur if the shutdown bypass is removed and both Main Feedwater Punps are trigxd.

OP-605, Feedwater System, will b3 revised to include this same " CAUTION".
