Description:

On 9/13/83,at 1300,a Recire Pump low low water trip relay (16Ak8A) was found to be smoking. The power to said relay was de-energized by the Operations personnel. Further investigation has revealed that this event caused a failure of one of the relay contacts, thus preventing the relay from performing its intended function.

Cause and Corrective Action: The most probable cause of this event is the smoke and varnish from the burned coil coating the relay contacts. The relay has been replaced and returned to service. The relay is a G.E. Model 12 HFA 51A 49F.
