/. y UPDATE REPORT - PREVIOUS REPORT DATE 08-01-80 ATTACHMENT 1 SURRY POWER STATION, UNIT NO. 2 DOCKET NO: 50-281 REPORT NO: 80-010/0lX-1 EVENT DATE: 07-21-80 TITLE OF REPORT: AUTOMATIC ISOLATION OF CONTAINMENT PURGE EXHAUST FAN DUE TO RADIATION MONITOR ALARM

1. Descrintion of Event:

While moving r6dioactive trash near room RMS 260, the monitor alarmed, causing containment purge to be tripped. All MOV's and supply fans tripped properly, but the purge exhaust fan did not trip. Retested operation with same results.

Ran test in Unit 1 with same result. This event is reportable in accordance with T.S. No. 6.6.2.a(1).

2. Probable Consequences and Status of Redundant Systems:

MOV's closed properly, thus if a fuel handling accident had occurred in containment , the isolation valve would prevent spread of contamination.

Possible consequence would be overheating of fan if fan was no, turned off manually. Unit 1 has been operating at power and no fuel handling had been performed.

3. Cause of the Event:

The Containment Purge Exhaust Fan power relay shown on drawing-11448-ESK-68H was bypassed as shown in Wiring Diagram 12846.21-EKS-76-1 during the .

accomplishment of Design Change 76-14B. The relay leads were lifted in.

anticipation of procedure 12 of Design Change 78-S34A. This procedure-prepared Fans 1-VS-F-5A and B (purge exhaust fans in unit 1 and unit 2) for removal during the accomplishment of " Auxiliary Ventilation Modification".

Design Change 78-S34A was approved by the Safety Committee, but was'not released by the Project Engineer _ in Richmond.

4. Immediate Corrective Action:

Established administrative controls to manually secure fan if necessary.-

No corrective action was .taken pending decisien of Project ' Engineer on j Procedure-12-of Design Change DC-78-S34A.

5. Subsequent Corrective Action:

A request to change the Technical Specifications has been submitted to the, NRC. This change reflects the changes accomplished. in Design Change 78-S34.

6. Future Corrective Action:

Not' applicable.

7. Generic Implications:
