ENCLOSURE 1 NOTICE OF VIOLATION ct W

During an NRC inspection conducted on March 16 through April 26,1997, a violation of NRC requirements was identified. In accordance Eth the " General Statement of Policy and Procedure for NRC Enforcement Actions," NUREG-1600, the violation is listed below:

Technical Specification 5.4.1.a states, in part, that written procedures shall be maintained covering the applicable procedures recommended in Appendix A of Regulatory Guide 1.33, " Quality Assurance Programs Requi oments (Operations),"

Revmr i 2, February 1978.

Appendix A of Regulatory Guide 1.33, Section 1, recommends administrative procedures for equipment control (e.g., locking and tagging) and bypass of safety functions and jumper control.

Section 1.8 of the River Bend Station Updated Final Safety Analysis Report states, in part, that the licensee will comply with the provisions of Industry Standard ANSI 18.7-1976, " Administrative Controls and Quality Assurance for the Operational Phase of Nuclear Power Plants."

Industry Standard ANSI 18.7-1976, Section 5.2.6, states, in part, that independent verifications shall be required in licensee procedures for tagging operations to prevent unauthorized repositioning and temporary modifications such as temporary bypass lines, electrical jumpers, lif ted electrical leads, and temporary trip settings.

Contrary to the above, as of April 26,1997, the three exam.m.,;; below were identified where procedures were not adequately maintained in that they did not require independent verification for the activities prescribed in ANSI 18.7-1976.

1. Operation Section Procedure OSP-0014, " Administrative Control of Equipment and/or Devices", Revision 7.
2. Administrative Procedure ADM-0031, " Temporary Alterations," Revision 7, did not implement independent verification requirements for procedures controlling bypass of safety functions and jumpers.
3. General Maintenance Procedure GMP-0042, " Lifted Leads and Jumpers,"

Revision 10, only partially implemented independent verification requirements I for procedures controlling bypass of safety functions and jumpers.

This is a Severity Level IV violation (Supplement 1)(50-458/97007-01)

The NRC has concluded that information regarding the reason for the violation, the corrective actions taken and planned to correct the violation and prevent recurrence and
