10, Code of f ederal Regulations, Part 26, Appendix A, Subpart  ;

Section 2. 8 (e) (4 ) , requires licensees to submit within thirty .i (30) days a report of any unestisfactory performance testing incident. The report shall ec dated and signed by the individual i responsible for the darr w-day. operation / management of the Department of Health and Hun.an Services (HHS) certified laboratory, and shall include the investigative findings and the corrective ,

i action taken by the laboratory.

Florida-Power Corporation (FPC) hereby submits 'three (3) reports of unsatisfactory blind performance testing incidents. The reports show that one incident occurred in January, 1990, and two occurred  ;

in. March, 1990. The results of three positive blind test saraples i (samples spiked with drugs for which FPC is testing) were certified  ;

' to contain no drugs. These errors were categorized as. " false '

negatives". Upon FPC's request, the' laboratory investigated the  !

errors, retested the samples, and confirmed the samples as positive  ;

using the gas chromatography / mass spectrometry. technique.  ;

The invettigative findings of the laboratory . indicated that the error occurred during;the initial. screening proce'ss. Based upon the laboratory's report, FPC.first determined the incidents were  ;

not reportable. After further_ review and discussions with the NRC,'

FPC is submitting copiw of the" reports as attachments- to this l letter.

Dear Dr. Kline:

The urine drug screen on the above~ mentioned patiant: -

originally screened as negative for amphetamines. Upon re-l peating the screening on the amphetamines, it was-positive.

The GC/MS confirmed the presence of amphetamines.

The reason this probably happenedLwas that in our I procedure these specimens are screened,.and if they are positive

for amphetamines, they are treated with' sodium hydroxide and i l periodic acid, and they are then rescreened. Unfortunately, at the time Chris-o' Kelly's specimen was.run, the pH was not checked on the specimens. . It is possible that the specimens ,

were not alkalinized sufficiently. Since you. called this to our attention, we are routinely documenting the ph on specimens ,

after treatment.
