Pursuantto10CFR21,thisletterisbeingsubmittedtoprovidewrittennotificationoftheexistenceofadefectassociatedwithmotor-operatedvalves(MOVs)suppliedtotheD.C.CookNuclearPlantbyHammel-Dahl6JamesburyControlsCompany,Inc.throughMidwestControlsCompany,Inc.Initialnotificationwasmadeinatelephoneconversationbe'tweenrepresentativesoftheAmericanElectricPowerServiceCorporationandMr.D.C.DiIannioftheNRCOfficeofNuclearReactorRegulationonAugust21,1987.OnAugust13,1987,duringtheinstallationoffourmotor-operatedvalvesintheAuxiliaryFeedwaterSystemattheD.C.CookNuclearPlant,metallicbeadswerefoundinthevalvemotorcasings.Thebeadsappearedtobeofthetypeassociatedwithanabrasivecleaningprocess.ThebeadswerefoundinoneofthemotorsduringinstallationofT-drainfittingsforthemotorcasings.Noisebeingmadebytheva"vemotorduringMOVATS(motor-operatedvalveanalysisandtestsystem)testingprompteddisassemblyofthatmotor.DuringthesubsequentinvestigationfortheremainingUnit1ACmotors,beadswerefoundintwoofthethreemotors.Thevalvemotors(575VAC)weremanufacturedbyRelianceElectricCompanyandsenttoLimitorqueCorporationtobeinstalledonvalveoperators(LimitorqueSMB-000).Theentiremotor-operatorassemblieswerethenshippedtoHammel-Dahl&JamesburyControlsCompany,Inc.,wheretheyweremountedtothevalves.Thevalves,withthemotoroperatorsattached,werereceivedbytheplantinMay1987andinstalledduringthecurrentoutage.Aftertheproblemwasdiscovered,itwasanalyzedbyAmericanElectricPowerpersonnel,andonAugust21,1987,itwasconcludedthatthebeadsfoundinthevalvemotorsoftheMOVsweresufficienttoL8709040009870826PDRADOCK05000315IiPDR
