REPORTABLEOCCURRENCE335-79-1ST.LUCIEUNIT1DATEOFOCCURRENCE:JANUARY23,1979TECHNICALSPECIFICATION3.1.3.1CEDM¹43TheattachedLicenseeEventReportisbeingsubmittedinaccordancewithTechnicalSpecification6.9toprovide30-daynotificationofthesubjectoccurrence.Verytrulyyours,A.D.SchmidtVicePresidentPowerResourcesMAS/cpcAttachmentcc:Director,OfficeofInspectionandEnforcement(30)Director,OfficeofManagementInformationandProgramControl{3)HaroldF.Reis,EsquireFile933.1SLv90818o&+9Dll7PEOPI.E..SEfIVINGPEOPLE riJt)t",lie(4"W1A REPORTABLEOCCURRENCE335-79-1LICENSEEEVENTREPORTPAGETWOAdditionalEventDescritionandProbableConseuencesWhenthereactorwastrippedaspartoftheshutdownprocedure,thetwoCEA'sactedonbyCEDM843stopped.during.theirfallintothecoreatthe8inchwithdrawnposition.Attempts.to:movetheseCEA'svereunsuccessful,andtheCEA'sweredeclaredINOPERABLE.Whileinvestigatingthecause,severalattamptstomovetheCEA's,verealsounsuccessful.Followingvendorrecommendations,CEDM843motorh'ousingwasremovedandinspected.*Themotorhousinginspectionresultsindicatedthatitwasnottheca~useof.theoccurrence.WhiletheCEDMmotorhousingwasremoved,amanuallifting.@evicewasattachedtotheCEDMdriveshaft.Byapplyinganupwardforce(equaltoaprox-imately15%morethanwhattheCEDMliftingdevice,normallyapplies)andusingphysicalagitationtotheliftingdevicetheCEA'swerefreedandexercised.TheCEDMmotorhousingwasreplacedandtheCEA'sandCEDMvereoperationallytestedsatisfactorily.CEAdrop.timetestingwasperformedasrequiredbyT.S.4.1.3.4b.Also,droptimete'stingvasperformedundercoldconditions..Alldroptimeswerewellwithinlimitswithnosignificantdifferenceswhencomparedtoprevioustestresults.TheCEA,'sweredeclaredOPERABLEafterabout79hours.Thereverenoadverseconsequencesduetothisoccurrence.Throughoutthe'periodinwhichtheseCEA'swereinoperable,adequateshutdown.marginwasmaintained.Virtuallynoadditionalreactivityinthecorewaspro-.ducedduetotheseCEA'sbeing8incheswithdrawn.BoronconcentrationintheRCSwasmaintainedsufficientlyhighenoughtoprovideadequateshutdownmargineveniftheseCEA'sandthenexthighestreactivityworthdual-CEA'swerefullywithdrawnsimultaneously.Also,CEA861droppedvhileinsertingrodsduringthissamereactorshutdown.AppropriateACTIONwastaken.Nocauseforthisdropcouldbedetermined.
