PORV's and Safety Valve Sugmary On February 1,1988 with Reactor Coolant System Temperature at 565 F.

Reactor Coolant System Pressure at approximately 2260 psig and the Pressurizer Pressure Controller in manual, Pressurizer PORV PCV-0655A was inadvertently opened for approximately two (2) seconds. The valve opened upon sensing a high rate of change of demand output from the Pressurizer Pressure Controller as it was being manually increased. Reactor Coolant System Pressure decreased to approximately 2207 psig before the PORV reclosed. RCS pressure was recovered manually using Pressurizer Heaters and the Pressurizer Controller was then returned to automatic, i
