SHIFT SUPERINTENDENT IMPIJ2ENTATION OF CHECKLIST

  • Complete Sections 1, 2 and 3 of this checklist if you have Emergency Direction and Control.
  • Complete Section 3 only, as instructed by the TSC Director, whenever the TSC Pg Director or EOF Director has Emergency Direction and Control.

e TSC DIRECTOR IMPLEMENTATION OF CHECKLIST

  • Complete Sections 1 and 2 if you or the EOF Director have assumed Dmergency Direction and Control.
  • Direct the Shift Superintendent to complete Section 3 following your decision concerning evacuation.

SECTION 1 - EVACUATION DETERMINATION NOTE 09 It is the intent of this procedure to evacuate personnel when any of the

' initiating conditions exists. Using extenuating circumstances to delay or abort an evacuation should be considered carefully.

1.0 Determine if any Initiating Conditions listed below are present

1.4 An uncontrolled toxic gas leak (originating either Yes No on-site or off-site) where the hazard is not confined to a local area.

2.0 Determine if any of the Extenuating conditions listed below, which might preclude immediate evacuation, are present (then proceed to step 3).

2.1 An ongoing security threat within the protected area Yes No (consult with the Security Shift Commander to aid in determining the safest course of action).

2.2 Inclement weather (e.g., tornado, high winds, hazardous Yes No road conditions that may preclude a safe evacuation of plant personnel).

2.3 Radiological or Hazardous Materials conditions exist Yes No which would subject evacuees to higher exposure / risk to s personnel safety.
