(1) Three (3) originals and thirty-seven (37) copies of an application requesting relief from Hydrostatic Inservice Inspection requirements.

(2) Forty (40) copies of Appendix 1 which sets forth a discussion of the relief requests, the code requirements, a basis for requesting relief, alternate examinations and attached figures to identify the I components affected. l A check in the amount of $150.00 is attached for payment of the application fee in accordance with 10 CFR 170.
