SUBJECT: NRC INSPECTION REPORT NO. 50-228/98-201 AND NOTICE OF VIOLATION
Dear Mr. Tsukimura:
This refers to the inspection conducted on November 2-5,1998, at your Aerotest i
Radiography and Research Reactor facility. The inspection included a review of activities i
authorized for your facility. The enclosed report presents the results of that inspection.
Various aspects of your safety program were inspected including selective examinations of ,
procedures and representative records, interviews with personnel, and observation of
activities in progress.
As a result of the inspection, one violation was identified. The violation is cited in the ;
enclosed Notice of Violation (Notice), and the circumstances surrounding the violation are
described in detail in the enclosed report. -

You are required to respond to this letter and should follow the instructions specified in the
enclosed Notice when nreparing your response. In your response, you should document
the specific actions taken and any additional actions you plan to take to prevent
recurrence. After reviewing your response to the Notice, including your proposed
corrective actions and the results of future inspections, the NRC will determine whether
further NRC enforcement action is necessary to ensure compliance with NRC regulatory l
requirements.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter
and its enclosure will be placed in the NRC Public Document Room.
